It's not uncommon for seniors to have several different Doctors treating several conditions. Everytime you see a physican they want to know your complete medical history and current medications.  Maristone Senior Living knows it's hard to keep up with.  That's why having a piece of paper by your side with all your history is so important.

It's called a Medical Information Sheet.  On it you should list all your doctors and their contact numbers.  You should include all your medical conditions, what prescription drugs you might be taking along with the current dosage.  You should write down your emergency contacts and insurance information. 

Make sure a copy is readily available at your home in a designated place where someone can quickly find it.  This can be critical if you are ever unable to communicate for any reason at any time.
